Part I of the Life Plan: A Person-Centered Planning tool to Develop Transition Services, the Functional Vocational Evaluation, and the Coordinated Set of Activities Virtual - 34178

The Life Plan is a person-centered plan (PCP) designed to identify post-school environments and activities aligned with post-school transition goals. Training content includes the identification of existing targeted skills through student and parent input, identification of instructional priorities, the levels of support, digital literacy, self-determination skills, and potential barriers in the adult world, such as, transportation needs, health literacy, and behaviors that close doors. Participants will learn the PCP concept, the evidence-based model of the Life Plan, and tools and strategies to implement the Life Plan. This session is aligned with the transition component of the TEA-required self-assessment and SPPI-13.

Objectives
After participating in the Life Plan training session, participants will be able engage students and families in the ultimate outcome of developing a comprehensive person-centered service plan that meets the needs and overarching safeguards and life goals of a students with complex needs.
